Wellness Check Monitoring
P721Q is committed to working with families to ensure the safety, welfare, and educational success of all our students. Accordingly, we are still monitoring and following up on engagement and attendance when one of our students is not consistently attending class either in-person or remotely.
When a student is absent, we will make a positive phone call home to check-in and make sure the student and family are healthy. During these conversations we will share information regarding the student's attendance and discuss how we as a school community can be most helpful in creating the conditions to help them attend regularly and thrive in school.
To keep a record of these outreach attempts, and when you are unable to collect IEP Data above due to attendance issues, please complete the wellness check form below to indicate if any progress has been document through asynchronous assignments and the strategies / supports that were recommended during your wellness conversations.
